Mulch Agreement

My name is ____________________________; I request that Tree Masters deliver free of charge,

___________ loads of mulch and/or wood, to the address stated above. In consideration for receiving this free
mulch/wood and it's delivery, I agree to receive the full contents of the truck, whatever it may be. Tree Masters
agrees that all wood (if any) included with mulch, shall be cut into lengths no longer than 30 inches. I understand
that under no circumstance will Tree Masters be responsible for the mulch or wood products after it is delivered. I
agree to hold Tree Masters, its employees and agents, harmless for any damages which may arise from the
attempted delivery or delivery of this product and that Tree Masters will not, under any circumstance pick up the
delivered material.

I understand that this is an open date order, and will be delivered, when we are in your area, at our convenience.
This order may be cancelled at any time, in writing only.

Note: Each load is approximately 3 to 5 yards of material.

Please draw a map as to where you would like our truck to dump. Draw the house, street and driveway then locate
where you would like the pile with an X. Please keep in mind that the truck is large and needs ample room to
dump. (Both height and width) Normal Driveways, are no problem, and are preferred.

If no map is drawn, delivery will be made as far back on the driveway, as we can maneuver our truck.
